H3G34-mutant hemispheric gliomas are molecularly and clinically distinct from H3K27M-mutant non-midline (hemispheric) gliomas despite sharing histone mutations; H3K27M-NDMG presents in older adults (median 45 years), shows comparable survival (~20 months), but has identical methylation signatures to H3K27M diffuse midline gliomas, necessitating location-independent molecular classification.
