Aducanumab ENGAGE (NCT02477800) and EMERGE (NCT02484547) Phase III trials halted for futility March 2019; subsequent post-hoc re-analysis showed EMERGE high-dose arm numerically positive while ENGAGE remained null. The ENGAGE-EMERGE discordance on identical protocols remains contested.
Reimbursement decisions: CMS National Coverage Determination (April 2022) restricted aducanumab Medicare coverage to clinical trial enrollment only — a near-zero coverage outcome in practice — citing insufficient evidence of clinical benefit despite FDA approval. Biogen withdrew aducanumab from market Jan 2024.
FDA accelerated approval of aducanumab (Aduhelm) June 2021 was contested at the FDA Advisory Committee (10-0 vote against, plus 1 uncertain); the surrogate-endpoint approval mechanism was used over Advisory Committee opinion. Three FDA AC members resigned in protest.
